Credit Claim Instructions - April 19, 2024

To claim credit, scan the QR code or follow the instructions below. 

1.      Scan the QR Code with your mobile device or visit: https://form.jotform.com/240845359535161

2.     Start the entire online Evaluation Process after your last session as you cannot save your progress.

3.      Claim only those minutes/hours of credit actually spent in each session.

4.      Before you submit your completed evaluation at the end, you will be prompted to include the total hours you earned. Refer to the worksheet. This will comprise your Credit Claim.

5.      You will receive an email from CMDA with your Certificate of Attendance attached – save it for your records.

6.     CMDA will report your earned hours to AMDA.
