Continuing Medical Education credit claim instructions

Instructions
- Claim credits as soon as possible, or before May 10, 2025.
- Scan the Credit Claim QR Code or click here: Credit Claim. (https://form.jotform.com/250393045647156)
- Complete evaluations for all sessions attended.
- Click Save at the bottom of the evaluation to save and continue anytime. A link will be emailed to provide access to incomplete evaluations.
- If you attended every session, you may claim the maximum hours (5.75 hrs)
- For partial credit, combine the number of minutes spent in all sessions and round to the nearest quarter hour to report the total credit claim. Refer to the conference schedule.
- Click Submit once the evaluation is completed.
- The Certificate of Attendance will automatically be emailed to you from CMDA.
- CMDA will report the earned credits directly to PALTmed.
