About the Learn & Connect 2026 Presenters

Leslie Beth Eber MD, CMD, FPALTC is board certified in internal medicine and is a Certified Medical Director.  She is dedicated to caring for patients as an attending physician with Rocky Mountain Senior Care. Dr. Eber is the president of PALTmed, the Post-Acute and Long-Term Care Medical Association and past-president of PALTmed-Colorado.  Dr. Eber was awarded the AMDA Medical Director of the year for 2021. She is committed to elevating the care for those with dementia through her work with the Colorado Dementia Partnership.

Greg Gahm, MD, FACP, Associate Clinical Professor of Medicine, University of Colorado, is a geriatrician and Medical Director for a number of Denver area nursing homes.  He attended the University of Colorado for undergraduate studies and medical school, did a residency in Internal Medicine in Arizona, a Fellowship in Administrative Medicine and Medical Informatics at Harvard and a sabbatical in Clinical Preventive Medicine at Stanford.  For many years, he led a monthly Geriatric Journal Club for LTC providers in Colorado, which is now known as the PALTmed-Colorado Learning Community.

With more than 15 years of experience, Leia Ginsberg, DNP, ANP-BC, has consistently demonstrated a dedication to clinical education and patient-centered care. She earned her first undergraduate degree in neuroscience and behavior at Mount Holyoke College and completed her nursing education at the University of Colorado Denver. In previous roles she has focused on quality improvement, developing educational programs that enhance caregiver skills and patient outcomes. Dr. Ginsberg’s commitment to empowering patients and honoring their values is central to her approach and current role with Bloom Health.

Rebecca Jackson, DO, CMD, FPALTC, is a geriatrician in Fort Collins, Colorado, currently serving as the Post-Acute Associate Chief Medical Officer (ACMO) for NCH Health Partners. Her experience as a medical director encompasses multiple settings across the geriatric care continuum, including inpatient rehab, skilled nursing facilities, assisted living communities, home health, and therapy services. Dr. Jackson is the President of PALTmed-Colorado.

As Chair of the Learning Community, Sing Palat, MD, CMD, FPALTC, brings a passion for geriatrics and education.  She is the Immediate Past-President of PALTmed-Colorado and the Market Medical Director of Home & Community for Optum Health. She is an Adjoint Associate Professor for the University of Colorado Division of Geriatrics. On weekends you may see her rounding with LTC Rehab Consultants or playing the piano.

Pradeep Raj Rai, MD, serves as volunteer faculty at the University of Colorado Health Sciences Center and is an experienced wound care clinician. He is the Medical Director of ProHealth One and Post Acute Medical Specialty Hospital of Denver, where he leads a comprehensive wound management program. With surgical training emphasizing burn care, Dr. Rai has worked extensively in long-term care since 2015.

Vikas Reddy, MD, pursued his medical degree at Spartan Health Sciences University School of Medicine in Vieux Fort, St. Lucia.  Dr. Reddy continued his training at the Central Michigan University (CMU) College of Medicine, where he completed his residency. His current role with Summit Medical consultants extends beyond direct patient care to include the education of non-physician providers and the analysis of organizational metrics to enhance healthcare delivery.  In recognition of his outstanding contributions, Dr. Reddy was honored with the 2023 PALTC Clinician of the Year award.

David Shepherd DO, MBA, CMD, FPALTC, completed his residency and a Medical Informatics fellowship at Indiana University. He has a passion for process improvement, antibiotic stewardship, reduction of polypharmacy, and maximizing the utility of the electronic medical record. In his spare time, he enjoys spending time with his wife and three children, playing guitar and drums, fly fishing, mountain biking, and serving as the Treasurer for PALTmed-Colorado.

Eric Yaeger, MD, is a board-certified pulmonologist based in Greenwood Village with over 35 years of medical experience. After earning his medical degree from the University of Colorado School of Medicine in 1987, he completed an internal medicine residency at Sky Ridge Medical Center and a specialized fellowship at the University of Arizona. He has specialized in treating chronic respiratory failure, published research in prominent medical journals, and was honored as the National Clinician of the Year by the American Thoracic Society in 2012.
